Tarlatamab + 抗PD-L1抗体

今週は越智先生が「Safety and activity of tarlatamab in combination with a DeLLphi-303 as first-line maintenance therapy after chemo-immunotherapy in patients with extensive-stage small-cell lung cancer (DeLLphi-303): a multicentre, non-randomised, phase 1b study. Lancet Oncol. 2025 Oct;26(10):1300-1311」を抄読しました。ES-SCLCの1st line（4-6 cycleのplatinum + etoposide + 抗PD-L1抗体）後の維持療法として、tarlatamab + 抗PD-L1抗体（atezolizumab or durvalumab）の安全性と有効性をみたphase Ib試験です。DeLLphi-303は多くのpartに分かれていて、今回はatezolizumab (n=48)、durvalumab (n=40)の計88人の報告でした。重篤なAEとして cytokine release syndrome 24%、pyrexia 7%、immune effector cell-associated neurotoxicity syndrome 5%、pneumonia 5%で治療関連死はなく、生存期間中央値は25.3ヶ月でした。安全性も有効性も期待できる治療でした。
