細胞診検体でのPD-L1発現

先週は、私が「Ming Sound Tsao, et al. PD-L1 Immunohistochemistry Comparability Study in Real-Life Clinical Samples: Results of Blueprint Phase 2 Project. PD-L1 testing using the clone 22C3 pharmDx kit for selection of patients with NSCLC to receive immune checkpoint inhibitor therapy: are cytology cell blocks a viable option? 」Am Soc Cytopathol. 2018 May-Jun;7(3):133-141.」を抄読しました。そもそもPD-L1発現の意義は細胞診検体では確立されていません。細胞診検体94例は、EBUS-TBNA (54/94; 57.4%)、pleural fluid (26/94; 27.7%)、fine needle aspirates (12/94; 12.8%)、bronchial lavage or bronchial brushing specimens (2/94; 2.1%)でした。同時期の組織検体138例と比較してほぼ同等のPD-L1陽性率でした。PD-L1>50%での1st lineのペムブロリズマブの効果は細胞診でも組織診でもほぼ同等でした。Blueprint Phase 2 Projectでも、細胞診検体と組織検体でのPD-L1発現は高い相関が得られており、細胞診での有用性が示唆されていました。
