先週の抄読会では三村先生がCRSについて解説しました。論文は「Cytokine release syndrome. J Immunother Cancer. 2018;6(1):56」、「The model of cytokine release syndrome in CAR T-cell treatment for B-cell non-Hodgkin lymphoma. Signal Transduct Target Ther. 2020;5(1):134」、「Cytokine Release Syndrome in Cancer Patients Receiving Immune Checkpoint Inhibitors: A Case Series of 25 Patients and Review of the Literature. Front Immunol. 2022;13:807050」、「Impact of Preemptive Use of Tocilizumab on Chimeric Antigen Receptor T Cell Outcomes in Non-Hodgkin Lymphoma. Transplant Cell Ther. 2023 Jul;29(7):429.e1-429.e6」です。Front Immunolの論文では、固形癌の25人にCRSを生じ、Grade 1–2 のCRSでは7人（28%）がICIのrechallengeをされていました。感染症などの鑑別を十分に行い、症状が完全消失（≦Grade 1）し、ステロイドを中止またはプレドニゾロン換算≦10 mg/日まで漸減でき、炎症マーカー（CRP/フェリチン）や臓器機能がベースラインに復帰していれば、慎重な再開は許容されるようです。
