EGFRとVEGFの両阻害

抄読会で私が最近のEGFRとVEGFの両阻害に関する論文4つ、①「Comprehensive characterization and clinical impact of concomitant genomic alterations in EGFR-mutant NSCLCs treated with EGFR kinase inhibitors. Cheng Y, et al. Lung Cancer. 2020 Mar;141:82-88. 」、③「Combined Inhibition of EGFR and VEGF Pathways in Patients with EGFR-Mutated Non-Small Cell Lung Cancer: A Systematic Review and Meta-Analysis. Peravali M, et al. Current Oncology Reports. 2020 Sep; 22: 119」、④「The Combination of Afatinib and Bevacizumab in Untreated EGFR-Mutated Advanced Lung Adenocarcinoma: A Multicenter Observational Study. Hsu PC, et al. Pharmaceuticals (Basel). 2020 Oct 23;13(11):331」をざーっと読みました。①EGFR変異には両阻害がPFS良い（NGS解析でのTP53変異でも良いが、TP53野生型のほうがさらに良い）。2nd-Generation EGFR-TKIは1st-GよりT790M変異少ない。②両阻害のほうがT790M発現が少ない。③両阻害はPFSは良いが、奏効率とOSは変わらない。④Afa+Bevaはexon 19欠失にもL858Rにも良さそう。西日本グループの8751R試験はT790M陽性に対するosime+bevaとosime単独の比較試験ですが、osime単独の方が良さそうでした。RELAY試験のram+erlo群でのT790M変異出現頻度は、erlo単独群と変わりませんでした。両阻害が本当によいかどうかはまだ不明です。
