大腸鋸歯状病変

J2の小田先生が「Endoscopic diagnosis of sessile serrated adenoma/polyp with and without dysplasia/carcinoma. World J Gastroenterol. 2018;24:3250-3259」を抄読しました。大腸の過形成性ポリープの中で、鋸歯状病変（sessile serrated adenoma/polyp: SSA/P）はBRAF変異、DNA修復酵素のメチル化、CpG islandのメチル化因子、MSI-Hなどが多い前がん病変であり、NBI像、拡大内視鏡像、病理所見を提示していました。最近ではSSL（Sessile serrated lesion）やSSLD（sessile serrated lesion with dysplasia）と呼ばれるそうです。熟練した消化器内視鏡医は病理像を想像でき、この前がん病変を見事に指摘するとのことでした。
